Forms We get it, these forms are annoying. We don't like filling them out when we go see the doctor. Unfortunately, within Ascension Health nationally, there are only a handful of doctors practicing Urogynecology. Since the conditions we treat are a bit specialized, we need some specialized data. Since our Electronic Medical Record was designed for a primary care physician, we have found the need to supplement the data. Please note, the link to the forms only allows you to download the forms. They cannot be submitted electronically. You must print them and fill them out. Our office can also mail them to your home.
Referral: You only need a referral if required by your Insurance (typically if you have an HMO). Our front desk is very good with knowing which patients require a referral, so please ask when you make your appointment.
Records: This is not always needed, but if you have records you would like reviewed by the physicians please try to obtain them prior to your visit. If the physician determines that records are needed at the visit, the office can send a release of records to obtain them after your visit.
You will be called or texted by our Electronic Medical Record service with a link to sign in and check in. This basically involves completing the system-required information and confirming your contact and insurance. This can be done when you get to the office, but make sure you give yourself a bit more time if you do not complete ahead of time. Please forgive us (we have no control), but you will get a robo-call the day of your visit to complete the COVID questionaire.
